I understand that I am participating in All Auras Touch (the Artwork) by Kate Mitchell (the Artist).

I acknowledge and agree that any personal information solicited, including my occupation, first name, last name, email, phone number and post code, when participating in the Artwork, will be held by Carriageworks in accordance with the Carriageworks’ privacy policy.

I acknowledge and agree that an image of myself will be taken, in the course of participating in the Artwork (the Image). I understand it is intended for the Image, which includes my likeness in conjunction with my solicited occupation, to be included in the display of the Artwork in an exhibition at Carriageworks from 08 January – 01 March 2020 (the Exhibition). Following the Exhibition, I understand that the Artist may use the Image, which includes my likeness in conjunction with my solicited occupation, for any purpose, present and future, in any territory worldwide.

I acknowledge and agree that all intellectual property rights in the Image taken by the Artist as a result of my participation in the Artwork are wholly owned by the Artist and I relinquish any and all rights in the Image resulting from my participation in the Artwork.

I acknowledge and agree that Carriageworks Ltd (Carriageworks) may use the Image resulting from my participation in the Artwork, including my likeness in conjunction with my solicited occupation, for any purpose, present and future, in any territory worldwide, without payment to me. The purposes for which Carriageworks may use the Image include, without limitation, the reproduction, publication, display and communication to the public for commercial and non-commercial purposes, including the following:

1. Exhibition catalogue
2. Press and review
3. Marketing and publicity, including social media
4. Exhibition graphics and signage
5. Education and interpretation
6. Carriageworks website and mobile apps

If I am an Aboriginal or Torres Strait Islander, I acknowledge that the Artist and Carriageworks, if made aware of my death, will take reasonable steps to ensure that the Image appears with an appropriate content warning for Aboriginal or Torres Strait Islander communities. However, I understand and agree that, despite those efforts, the Image may still be displayed, published or disseminated.
